Not if you're on tourism, most people do speak English, but if you're there on other purposes and want to do paperwork, for example, you might well have a hard time, yes. You will also be at a disadvantage against other people who speak German when looking for a place to stay.
It is doable, depending on if you find a college that teaches in English, but you need to prepare a little in advance for it and, ideally, have someone who can help you when you're in a situation where German is mandatory.
