About buildings example, most recent buildings(in Lebanon) is still highly incompetent in planning of cabling and etc. For example in Russia for tens of years each multi-tenant building has special cable channels for low-voltage and high-voltage lines, and there is particular strict standards how it should be done for each type of cabling.
It includes for example 32 cabling categories, including for example (rough translation) "Cabling for sound beacons for vision-impaired people to locate their entrances", and this categories have sub-categories, like "local automation systems", has subcategory N7 - "waste removal automation system".

Even with that there is often fu**ups from ISPs, but at least it is possible to kick them and force to do things properly.
So i believe major reason is not order of things, but lack of proper laws, standards and huge incompetence.
As i said about, let them give permission for someone to make that in private sector, develop standards at least, and you will have superb internet in 1-2 years in most of areas, without any silly excuses and lies.
